AKNE TRUNKAL: TANTANGAN DERMATOLOGI YANG KERAP DIABAIKAN

Akne vulgaris merupakan salah satu gangguan inflamasi kronis pada unit pilosebasea yang sering dijumpai. Cutibacterium acnes (C. acnes) merupakan bakteri utama yang berkontribusi dalam proses patogenesis penyakit ini. Lesi akne seperti komedo, papula, pustula, hingga nodul yang dapat muncul di berbagai area tubuh, termasuk wajah, leher, serta batang tubuh yang dikenal sebagai akne trunkal. Sebanyak 61% pasien akne vulgaris juga mengalami keterlibatan area trunkal. Walaupun patogenesis akne wajah dan akne trunkal secara umum serupa, adanya perbedaan karakteristik anatomi dan fisiologis kulit trunkal diyakini dapat memengaruhi respons terhadap terapi. Secara teoritis, akne trunkal dapat ditangani dengan pendekatan terapeutik yang serupa dengan akne vulgaris pada wajah Namun, kepatuhan pasien terhadap terapi topikal sering menjadi kendala, terutama karena luasnya area lesi dan kesulitan dalam aplikasi obat. Oleh karena itu, pemahaman yang komprehensif mengenai tata laksana akne trunkal sangat penting, mengingat terapi sistemik seperti antibiotik oral sering digunakan sebagai alternatif, meskipun penggunaannya harus dilakukan secara hati-hati karena risiko peningkatan resistensi antibiotik.
